Lab Clearance/Closeout Inspection Request

Lab safety personnel must conduct a clearance or closeout inspection anytime a lab is vacated. Please refer to the Lab Closeout Guidelines before requesting your inspection. A checklist is provided to insure that proper procedures have been followed. If you have waste chemicals that will not be moved, please complete an online waste pickup request. When you are ready please complete the information below and submit to request an inspection.
